Can't answer your questions, but can point out that the actual paper size is often considerably larger that the plate size that is actually printed on that paper. A remarkable number of oversize editions can be photocopied (when that is legal, of course) on U.S. letter size with very little or no reduction, because the plate size fits those dimensions. The obvious question, then, is why oversized paper was used?
