Which of these two notations is better? Or are they both wrong?

In my view, neither one is wrong. However, the context within which the fragments occur might make one a better choice than the other. My general rule of thumb is that the most critical information should be positioned so that it is perceived first, so in this case, if the accidental is the most critical information the performer needs, then the top line is preferred. If the top two musical lines are of equal importance, then the bottom version would be better. And if the outer two lines are more important, then I'd invert the stems on the middle notes, and displace the notes to the right of the beat axis.
