Hi Rich --While I would agree with everyone else that for most contemporary music it's not common, it IS common in musical theatre -- most computer-engraved (but not hand-engraved) scores I've come across (especially from MTI) in recent years include the name of the copying service, if not the copyist him/herself, on one of the first pages.
