What I thought it was you would have seen in 2010 after making the jump from 2008 but it sounds like everything was fine in 2010 then things broke in 2012.
The issue I was thinking about has to do with custom staff lists pre-new expression model. Going from the already broken model in 2008 into the "modified" broken model in 2010 and 2012 creates the same type of problems you describe. If you extract parts from this score do some of the blank ones show up in the parts? Also, in the score check if the blank ones have "Parts Only" selected from the pull-down menu in the expression's dbx when you shift-return the handle. If so, this is a good sign that some of the BAD staff list issues came over from your 2008 file. There is a way to avoid issues like this but you have to know what to do with the 2008 files first in changing the staff lists BEFORE you upgrade the file. PITA! Once the damage is done it's usually easier to just replace the items and be sure to delete the old ones from the expression dbx or you will have duplicates or new ones showing up when going to parts. This is the worst part about this bug because your score may look fine but when you get to parts you may find you have to delete or add a lot of items.
