I don't know what computer you're using, but on a Mac you can create a custom page size when you select the "manage custom sizes" option under "Paper Size." In Preview, go to "Print" - "Paper Size" - "Manage Custom Sizes." Under that heading you have a window listing existing custom sizes, which might be blank if you don't have any. At the bottom of that window is "+" "-" and "Duplicate." Select the "+" and enter all of the settings - Width, Height and Non-Printable Area. Hit "OK" and you're done.
