Linda,

I believe that the Tempo Indications category of Staff Expressions is set to 
only attach to the beginning of measures. Change the category settings, or 
create a miscellaneous Staff Expression with the Playback set to your tempo 
change, and that should allow you to attach it where you want.

Can't help you with the MIDI map question, but an effective workaround for 
creating ritards is to use the Tempo Tap tool, available in Studio View.
