I read something on the Apple Discussion Boards about the problem where an app turns into a folder, and their fix (of course provided the folder has the .app extension) was to delete the three files in your ~/Library/Preferences that start in 'LS' (i.e. rm ~/Library/Preferences/LS*). Then restart, which will recreate these files which apparently manage extension associations.
