I use fvwm as a window manager and
1/ I always have the 'pager' open, so I can control-click there
2/ I've told it to open an xterm automatically if I go to a desktop 
where there isn't one yet.

But that last one probably requires a he-man window manager, and not 
that sissy stuff like windowmaker or icewm :-)
