I suggest you look at the log files for mysql that will (should) be in /sw/var/mysql/
and see if they tell you anything useful.
Alternatively, try starting just mysqld instead of mysqld_safe and see if the same think occurs. (If the server does come up and stays up, you'll need to use mysqladmin to shut it down again.)
