I am not sure what image file you mean, the xcode disk image from Apple? In this case, X11SDK.pkg is probably in a folder named "Packages", and it is a standard Apple installer package, to be installed by double clicking on its icon. You shouldn't need to worry about where to install it, it chooses its place itself. Most of its files will go into /usr/X11R6 and, what is probably important for your case, there will be a symbolic link /usr/include/X11 -> /usr/X11R6/include/X11 But all of this should appear automatically.
