There's also some kind of problem with xterms that fits this
description. It can happen if you close xterms with cmd-w or the red
close button rather than typing 'exit' in them. Cases (a) and (c) may
be identical but you're not fast enough to catch the xterm exploding,
or if not this only explains (b) and (c) and the first one remains a
mystery.

In case (b) you may find you can open another xterm and kill the
offending one with xkill.

There's unfortunately no proper solution to this bug that I've come
across

amongst other places this problem has raised its head. (Most of the
'solutions' there do not work or simply do not apply, eg my xterms
which suffer this problem have never been login shells. Using Dave's
luit suggestion also makes no difference to me)
