You can install the *-shlibs packages of all 3 variants simultaneously. Only the *-dev packages are mutually exclusive. But this will only make sense if you want to use the X11 version of vtk (It does make sense in fact, because the Tk windows are actually working in the X11 and Carbon variants, as opposed to the Cocoa variant). It seems that by default VTK does deliberately not build Tk bindings when COCOA is ON.
