I've just solved the problem. It turns out that my "new empty" firefox profile was not actually empty---some vendors seem to have snuck their extensions into being included in new profiles by default. The particular guilty party is the skye extension, version 2.2.0.102. When this extension is enabled, firebug's menus are available but completely unresponsive. Disabling the skype extension brings firebug back to life.
