I think the per-url model works fine the majority of the time, but like Nicolas, I'm dealing with a special case right now where it just doesn't work. I propose adding a "Always enabled" option that forces Firebug to be enabled for every page no matter the domain or url. You can turn it on when you run into a special use case, and if you turn that off, it goes back to the per-url model. This would solve my issue, and I think it would be fairly simple to implement.
