I am still seeing similar stuff. Even stranger, when it working OK, if I switch tabs every item in the stack trace turns to [xpconnect...tackFrame]. If I switch tabs again, it changes the list to a single [xpconnect...tackFrame]. Things are hosed at that point and time to restart Firefox.
BTW: I found that if I have "Omit Toolbar Stack" (BTW, what does that mean?) *disabled* then I often will be debugging comments not code at some point (that is, it is way off on line numbers).
