I guess an extension to Firefox which is not extremely popular nor extremely obscure is causing a Firebug error during reload, so it fails to reopen. This guess is based on about 10-20 reports of similar problems. If we guess that only 1% of the people with this problem report it, that means 2000 users out of what, something between 250k (last weeks downloads) and 1.6M (last weeks pings). That is why I say the problem is rare but not extremely so. Firebug normally when the window is destroyed. So that part is ok. But Firebug should reopen, it fails to do this for you. So I guess some error is stopping it. (Firebug 1.4.1 will not close on reload, but that won't help you, it will be open but broken for you). You could try 1) Firebug Icon Menu (upper right) > Options > Reset All Options Or 2) Install 1.4X.0 from http://getfirebug.com/releases/firebug/1.4X, and use the tracing system to look for the error.
