Very odd, I disabled all my addons and plugins, and then turned them on
one by one, to find the culprit.. but now they are all on and i'm not
seeing it anymore..
Bug squashed i guess :)

Having Firebug installed and having the web console open, opening a
popup window, adds a nested firefox orange menu.
I can't reproduce this issue. Could that be a conflict with another
extension?
