I'm encountering the exact same issue on Firefox 31 ESR (31.2.0) on RHEL
6.6. No toolbar button, F12 brings up the Firefox developer tools, no way
to add the toolbar button back, no option in the Tools menu.
Installed FBTrace and disabled all other addons besides that and FB and
still no button. I did get some errors in FBTrace:
