Try looking into IPCHAINS IPPORTFW facility.  you'll need 
to run a 2.1.103+, or 2.2.* kernel, but it can forward 
any port to a different machine on the same or different 
port, and do it transparently to the application.  I'm not 
sure it handles UDP, but I don't see why it couldn't.  I use 
it for my web server.  Take a look at the IPCHAINS HOWTO for
further information, and web sites to check for instructions
on how to use.
