I tried that, went back and read the docs again, and still have not fixed
the problem. I think it is one of the many post service pack patches. I'm
going to do a clean install with (only) the required patches and see what
happens.

Make sure that transparancy is turned on on the interfaces.
Also make sure that for your NAT that the interface to see global addresses
is set to external untrusted interface.
