Just have a look at the TIS (Trusted Informations System) Toolkit.
It's named: Internet Firewall Toolkit (FWTK)
It has been long in use and has been taken as a framework for further
development of other proxy servers.
Have a look at www.tis.com.
You also could have a look at the socks5 source code.
