As Mikael said, the Exchange Server will authenticate against a DC,
but that happens behind the FW. If you leave the Outlook profile
configured for 'NT Password Authentication', then Outlook will send
the credentials of the currently logged on user to Exchange (through
one of the discussed port). If you switch the authentication to
'None', Outlook will prompt you for username, domain and password,
which is again transferred through those ports to the Exchange
server.
