Source Address (IP source)
Source Port (or ICMP Service Number)
Destination Address
Dest Port
Action (Accept/Deny)
NAT'able (Yes/No; basically, does NAT break this service)
Proxy Notes (Does a proxy exist; if so, where can you find info on
implementing it)

That should be a start; I am willing to volunteer my services in any
way you need it; I code ASP's with vb and/or perl, or just general
research or whatever.
