It looks like it's Peter da Silva's "plugdaemon".
Basic command line is
plug source destination
Relevant arguments:
-l turns on logging
-i addr binds to the interface with that address
so, your first plug listens on 195.164.40.75 port 12000, forwarding the
traffic to 205.57.240.71 port 12000. The second listens on port 119 (nntp)
on all interfaces, forwarding to 192.16.13.141.
