It is an applicatin with some sort of support from the Kernel. Linux and BSD
feature this. you can run an application and bind to foreign addresses. Of
you can even redirect every connection to a port to your application. Your
application can then use default networking API to request the actual
address.
There is a small transproxy tool and there is a transparent proxy patch for
squid.
