I've been using it for over 4 years on large-scale production projects for clients like Sunkist, Shea Homes, The Irvine Company etc... a lot of high-traffic sites with a lot of concurrent users. No problems, ever. And like Mike mentioned, the PHP code itself is really quite simple, if you don't like anything about it, just change it - I have on several occasions. I would highly recommend it.
