Well, I can't find any existing code for this...
So the possible way to go is:
1) create new textfield above the existing one
2) Fill it with one big initial letter
3) Strip the first letter from the main text
4) Place some IMG tag below the cap OR use indents...
5) fix all issues and enjoy ;-)
