I started off with a no-name combo USB/FireWire card with VIA chips from 
the local computer store. As far as I could tell, it was working fine, 
but after reading the recommendation to use a card with Lucent or Agere 
chips, I found an Adaptec AFW-4300C FireWire-only card with an Agere 
chip in my junk box. But it's likely that Adaptec -- just like other 
manufacturers -- changes chips like most people change socks, so there's 
no guarantee that an Adaptec FireWire card today uses an Agere chip. 
Best bet is to eyeball the card before you buy.

I'm using WinXP. I have no idea whether it works with Vista.
