What is happening actually is a problem with David's first case and a set of
circumstances that seems to be repeated in a number of cases. The page gets
renamed to some nonsense (but usable name). The system creates a redirect
page for the original name to the new nonsense name (and disconnects history
for the original topic name). Someone tries to rename the topic with the
nonsense name back to the original name, but is not allowed due to the
redirect page having the original topic name. The redirect topic is
unreachable as long as the topic with the nonsense name exists due to the
auto-redirect, so it cannot be deleted.

At a later point in time someone else sees this page with a nonsense title,
reaching it without going through a redirect, then thinks it is just a play
page that has been copied and subsequently deletes it. Now the redirect page
can be deleted and most often is as it now goes nowhere (and all the history
disappears). The content is now gone entirely from the wiki.
