Sorry I wasn't clear. Yes, I mean #1, the .jpg or .doc or .mpg are the "bulk data" that could go in another location, while the metadata remain in the wiki namespace.
I suggested this option since Craig mentioned, what if the attachment data was in its own directory? I thought, well, as long as the .jpgs are in another directory, what if we allowed that directory to exist in a separate path altogether that can be configured by the administrator? One reason this would help us, is that we may want to backup or manage attachment data separate from the wiki text, since one is luch larger than the other (particularly if we embed movies which is quite likely given some of our use cases).
