I'm running an NVIDIA card on Mandrake 9.0. Try this.

1.) make sure you are using XF86Config-4

2.) it should have a device section something like this (here's mine)
Section "Device"
    Identifier "NV AGP"
    BoardName "NVIDIA GeForce4 (generic)"
    Driver "nvidia"
    BusID "PCI:1:0:0"
    Option "DPMS"
EndSection

3.) The Modules section has:
       Load "/usr/X11R6/lib/modules/extensions/libglx.so"
       It's a symlink pointing to libglx.so.1.0.3123

3.) my etc/modules file has one entry: NVdriver

4.) my etc/modules-conf file has an entry: alias char-major-195 NVdriver
