The position is the cartesian coordinate vector in the WGS84 earth centered coordinate system. And the orientation quaternion is that one that rotates a vector in earth centered coordinates into the body coordinate system. Multiplying quaternions means successive rotation. I would guess that you get the orientation of the body wrt the horizontal local frame by premultiplying with the inverse of the orientation of the horizontal local frame. Which means 'first rotate from the horizontal local to the WGS84 then rotate from the WGS84 to the body'. Hope this helps.
