For the United States, two IAP formats are common, NOAA and
Jeppesen. David mentioned a URL for NOAA plates; You can see some
examples of both here:

Jeppesen and NOAA plates differ in layout and in some cases in what
information they provide. Both are legal to fly IFR. Personally, I
prefer the Jeppesen layout, but it's a matter of taste.
