Hi Sarah, Ironically I saw that yesterday for the first time and was confused myself. I traced it back to some testing we were doing with different flow-masks. Turns out IOS v 12.1(12c)E2) does not have the "set mls flow interface-full" command I was hoping would allow us to see input and output interfaces. So I experimented with "set mls flow destination-source" which turned off port number collection and they were all zeroes (both src and dest).
I suspect it is your flow mask setting. Note (from Cisco) : "12.1(13)E - This command was changed to add the interface-destination-source and interface-full options."'
