I've mainly used gnu "indent" for that, and it is available for OSX too. However, despite using it on and off for many years, I still don't understand all the options it takes. It is very configurable though, if you can figure out what to ask for, and it does a decent enough job. I think indent has options that allow you to control how it munges comment blocks, so can probably be made to work.
