This review looks awesome. One question, after reading the notes: what type of 
video were you captioning? A YouTube video, or from Vimeo, or in H.264/WebM 
player? I'm thinking it's worth making note of that, and also trying a few 
different formats to see how they compare. I'm guessing the video player being 
used will have an impact on the overall experience. For example, your note 
about the video being a focus trap in Firefox makes me think you were probably 
testing with a Flash-based player of some kind, since this is an inherent 
problem with Flash on non-IE browsers.

It sounds to me like several of your observations could be pretty 
straightforward to fix. Is now a good time to start working Dean, Craig and 
others to get some patches in?
