FYI - from NCAM - the demos work ok, except that since I'm not an expert screen 
reader user my speech rate is set much lower than the timing assumptions built 
into the video pauses in the demo. This causes audio overlap between the video 
and my screen reader. This is potentially something that could be built in 
personal preferences.

Apologies for the cross-posts.

Not long ago the National Center for Accessible Media at WGBH (NCAM) announced 
the availability of some demonstrations how the Popcorn.js HTML5 Media 
Framework can be used to deliver screen-reader accessible supplemental 
materials alongside video embedded in a Web page.  Our two prototypes-- 
http://ncamftp.wgbh.org/rd/hgp/ (which demonstrates accessible text and images 
alongside video) and http://ncamftp.wgbh.org/rd/quadratic/ (which demonstrates 
an approach to accessible math)-- have recently been updated.  On both sites, 
please read the instructions and system requirements, then watch the videos to 
see (and hear) accessible Wikipedia entries, glossaries, images with long 
descriptions and math displayed as the video progresses along its timeline. 
These demonstrations were made possible with funding provided by the Reader's 
Digest Partners for Sight Foundation.
