are these all the same, or are there indeterminate instructions for its
manufacture?  i think the event you're thinking of is simply a page who's
corners are folded in, which definately recalls the idea of envelope.  i
remember reading an interview with maciunas in ken friedman's book which
made reference to a letter and envelope set he collaborated on, the letter
shaped like a ahnd and the envelope a glove.
