In The Worlds of Nam June Paik by John G. Hanhardt, the catalogue from the Paik retrospective at the Guggenheim Museum, the following caption appears on page 75:

Zen for Film, 1964, performed as part of New Cinema Festival I, Filmakers' Cinemateque, New York, November 2, 1965.

I don't know if (or where) it was performed in 1964.
