Do you just need to set the input and the output to "aggregate device"
in the sound settings - or is there more to it than that?
yes, you have to make an aggregate device in audio midi setup, i think 
it is explained in the jackosx manual. apart from that i don't think 
anything else is needed. it is a common question, so we should write a 
chapter on this topic in the manual.
