I find that often as well - "export with current layout formatting"
rarely does. Other option is to generate calc fields based on
exported fields and set each as _ctName = Upper(Name) and export the
calcs instead. That tends to be more reliable - quite so in fact.

Have you tried turning on the "Export with current layout's
formatting" option?
