Changing the <fo:region-body>'s bottom margin fixes the problem. Here's the change made to one simple-page-master: BEFORE: <fo:region-body margin-top="3cm"/> <fo:region-before extent="3cm"/> <fo:region-after extent="1.5cm"/> AFTER: <fo:region-body margin-top="3cm" margin-bottom="1.5cm"/> <fo:region-before extent="3cm"/> <fo:region-after extent="1.5cm"/> This is because the region-after sticks up into the region-body by the amount specified in the extent property.
