This bug doesn't depend on the break-before/after attribute. The problem seems to be (for me at least) that fo:page-number-citation does not output the last referenced page number (e.g.: in a TOC). Another workaround for this is to print a dummy fo:page-number-citation right after the last TOC-entry.
