I will upload a second test case that shows a more real-world example, closer to what I'm struggling to obtain. This new test case (that contains both the header and a sidebar) exhibit another bug: the header and footer are misplaced by the presence of the sidebar, something that doesn't happen with version 0.20.4. I upload this second test case in the form of a couple of XML/XSL files because I think they're easier to read than the resulting FO file.
