I guess you use the -xml xxx -xls yyyy options so you never see the "real" fo-inputfile. My experience is that the build in (or should i say internaly called) xsl processer is sometimes funny. I can only suggest to use an external xls processor like xsltproc or xalan. After having problems with the build in xls processor I switched to xsltproc which worked fine for me.
