I see nothing wrong in PDF produced with the provided test case:
only 1 page is output,
And I get both Header and Footer that should come on 1st page, using FOP 0.95.

IIRC, FOP 0.95 did not manage correctly the page-position="rest", but that was
corrected in subsequent release.
Note also that when there is concurrent fo:conditional-page-master-reference,
the 1st is taken into account; so, in your case (single-page), both "first" and
"last" are eligible, but since the "first" is declared before, it is taken into
account.

Feel free to reopen if the issue persists.

No problem. Just keep in mind that FOP handles only FO input, and more it is
minimal, quicker will be the response from contributors. So anything that can
help everybody to give a precise answer is welcome. Better is to ask on
FOP-users list before, because the audience is larger than for bugtracker.
