Two observations:
(1) the overhead of implementing a new extension element is much greater than
implementing a new extension attribute;
(2) wrapping multi-switch in an extension element may cause that content to be
ignored in a processor that doesn't recognize the extension element; in
contrast, adding an extension attribute (property) to multi-switch without
enclosing it in an extension element will permit better interoperation with
processors that don't recognize the extension; for example, the user of a best
fit extension may wish to specify starting-state="show" on some multi-case to
serve as a default in order to select some content; this probably wouldn't work
when wrapping multi-switch in an extension element;
