Your FO refers to a document you've not provided - PDF/input.pdf
With an FO configured to include original.pdf as as an external-document, I
cannot get FOP 2.8/fop-pdf-images 2.8 to produce the output you describe. Are
you using an fop.xconf FOP configuration file?
