Update: It appears that including a PDF/UA compliant document as an image and
producing another PDF/UA compliant result works if and only if the image is the
last content in the source FO. If there is any more content after the
referenced PDF, the result is not PDF/UA compliant.
Including a PDF/UA compliant document via fox:external-document does not work
for me in any case.
