Looks like the behaviour of <fo:leader> has changed. The following used to
put a 3pt rule across the page (taken from leader.fo):
<fo:block><fo:leader leader-pattern="rule"
rule-thickness="6.0pt"
leader-length="100%"/>
</fo:block>
